A program for heart or lung patients that includes exercise, education, and guidance from our specially trained staff.

Diet, exercise, smoking, and stress can have a significant effect on your health. If you have a heart or lung problem, or are at risk for cardiac disease, lifestyle changes are especially important to you.

But it can be difficult to make changes. They’re much easier said than done.
We can help…

Upland Hills Health offers specialized cardiac and pulmonary fitness programs. They include exercise, education, and guidance to meet your individual needs and goals.  Your very own staff of expert, personal trainers works with you to develop your comprehensive, individualized program.

Getting started…

Talk to your family doctor, pulmonologist, cardiologist, or cardiac surgeon about referring you to our Cardiac and Pulmonary Rehab program. Cardiac Rehab is prescribed for patients who:

  • have had a heart attack
  • have had bypass or coronary valve surgery
  • have chronic, stable angina pectoris
Pulmonary Rehab is prescribed for patients who have chronic pulmonary disease, such as:
  • chronic bronchitis
  • emphysema
  • asthma

Patients with other heart or lung conditions also may be eligible for Cardiac or Pulmonary Rehab.  Talk to your doctor to see if you qualify.


Here’s how it works…

You’ll come to our Cardiopulmonary Exercise Facility 2-3 times each week. One-hour sessions take place in a safe, monitored

environment, where our staff will observe your heart rate, blood pressure, heart rhythm, and/or oxygen levels, and evaluate you for any signs or symptoms of heart- or lung-related changes.

Should they be needed, our emergency physician and emergency nursing staff are readily available 24 hours/day, 365 days per year.

You will learn about "healthy" living, including tips on risk factors, stress reduction, medications, healthy eating, exercise, and smoking cessation if applicable.  You’ll have the opportunity to work with other patients, share their experience, and motivate each other to achieve the lifestyle changes you choose.

Your physician will be kept informed of your progress.

Who might benefit from Cardiac or Pulmonary Rehabilitation?

The Cardiac and Pulmonary Rehabilitation programs at Upland Hills Health Inc. are designed to help those who wish to:

  • Return to work.
  • Improve exercise tolerance.
  • Have more energy, and less shortness of breath, for performing daily personal, home and leisure activities.
  • Prevent or decrease their risks associated with heart disease after they have had heart injury or surgery.
  • Manage and live with their lung disease.

What makes our program special?

Our Cardiac and Pulmonary Rehabilitation program brings a multidisciplinary team consisting of registered nurses, physical therapists and nutritionists to serve you.

Assessment and exercise sessions are lead by critical care nurses who understand the complex functions of the heart and lungs.  These nurses have been trained in exercise therapy to help you recover functionally and emotionally after a cardiac event, heart or lung surgery, or maximize your activity potential living with chronic heart or lung disease.

Our Cardiopulmonary Rehabilitation nurses will closely monitor each step of your therapy. They can identify any potential problems, and communicate them along with your progress to your physician on a regular basis.

Financial questions?

Before your first visit, if you desire, our staff will contact your insurance company or other third-party payer to discuss your plan of care and to help coordinate your reimbursement. Our Cardiopulmonary Rehabilitation program is covered by most insurance programs and by Medicare.
