About the Upland Hills Nursing and Rehab Center

Constructed in 2002, the Nursing and Rehab Center was designed to enhance the long-term care needs in the community and to replace a physical plant on North Street, which maximized its usefulness. The new construction was one of the first of its kind in Wisconsin — designed with the "household concept," supporting a homelike, rather than institutional living environment for the frailest seniors and others in need of a nursing home environment in the greater Iowa County area.

The project includes a new 44-bed long-term care and rehab facility, designed in a "neighborhood concept" with four large homes or households. Each household has a living area, dining area, parlor, nine bedrooms, and nine bathrooms (for 11 residents per household). The four households connect to a "neighborhood center" for group activities.

The Skywalk, known as "Main Street," has a beauty/barber salon, post office drop box, ATM, movie theater, chapel/meditation room, library, greenhouse, conversation and viewing area, and a computer services area.

Residential Household

The Residential Household was created for residents who are cognitively alert and aware, but who are unable to either care for themselves at home or live in an assisted living facility. The household consists of 7 private and 2 semi-private rooms, in addition to a den, parlor, kitchen, dining and living room. There is easy access to the outdoors where residents enjoy the beauty of an outdoor courtyard. Activities are held regularly in the Residential household, from baking to word search. There is always a puzzle going in the den or a game of cards around the table. Families visit often and spend time in the Residential Household or on Main Street, enjoying the café or bird watching in the Aviary.

Rehab Household

The Rehab Household was created to meet the needs of persons who have recently been hospitalized and needed a re-couperative stay to enable them to return to their prior living situation. Frequent diagnosis in this household are orthopedic procedures such as hip or knee replacement, rehabilitation following a CerbroVascular Accident, or medical recovery from diagnosis such as congestive heart failure, pneumonia or other infectious disease.

Staff in this household is trained in exercise therapy, special positioning and transfer skills, and monitoring of sub acute medical disease status.

The Rehab Household has consistently received extremely high patient satisfaction scores from residents who have received their care and services and have returned to their prior living location.

Nursing and Rehab Center added to top ten list

Our nursing and rehab facility has received the honor of being listed in the "Top Ten" Most Innovative Facilities in Healthcare. The Center for Innovation in Health Facilities is a non-profit organization created in 1997 to foster innovation in evidence-based design, planning, and operation of healthcare facilities. The Center conducted a survey asking for information from 5000 healthcare CEOs and 250 healthcare consultants about innovations in healthcare environments. This survey resulted in the "Top Ten" Most Innovative Facilities for 2000, of which, our organization was named for the state-of-the-art nursing and rehab facility that is currently under construction.
