To apply for volunteerism, download this pdf format Application Form. Print the form, fill it out and mail it to the address indicated on the form. A representative from Community Relations will contact you to discuss your application and volunteer opportunities.

The Upland Hills Auxiliary is another great way to give of your time and talents. If you'd like to help with fundraising events, please download this pdf Membership Coupon. After completing the form, please mail it with your membership dues to the address indicated on the form. You will be added to our Auxiliary membership list.

Volunteers are very valuable to Upland Hills Health. They are honored each year with an Appreciation Banquet, as well as other activities. Volunteers are also offered discounted memberships at our Therapy & Wellness Center and free immunizations.
