Diabetes Self Management Education

Almost 10% of American adults are living with Diabetes. Although Diabetes is a serious disease, many people live long and healthy lives by learning to manage their blood sugars through lifestyle changes and other self management skills.

Our Diabetes Self Management Program (DSMP) is available for people diagnosed with Type I and Type II Diabetes. The program provides comprehensive diabetes education for mastering the basic self-care skills for your lifelong journey:

  • What is diabetes?

  • Basics of nutrition

  • Carb counting and meal planning

  • Navigating the grocery store

  • Blood sugar monitoring

  • Understanding medications

  • Sick day management

  • Managing long term complications

  • Healthy coping strategies

  • Benefits of exercise

Our DSMP is accredited by the American Diabetes Association and is taught by our Registered Dietitian and Certified Diabetes Care and Education Specialist, Jackie Wilkinson. The program consists of up to 10 hours of education. For more information on the DSMP, please call 608.930.8000. A referral from your Primary Care Physician is required to enroll in the program.
