What if a sudden illness or injury left you unable to speak for yourself?
Who would you want to speak for you?
Does your doctor and family know your wishes for future medical care?
Most people say it’s important to talk with your family about their future medical care and to put it in writing, yet only a few people do. At Upland Hills Health we’re here to help you take the next steps to complete a written plan. We’ll help you:
Think about your health care values or goals
Talk about your choices with your doctor and loved ones
Make a written plan, called an advance directive. This will include you choosing the person you want to make health care decisions when you’re unable to communicate with the doctor.
Wisconsin law requires this designation be in writing. Give your loved ones peace of mind by talking about your medical care wishes and completing a written plan. Call Upland Hills Health Patient and Family Services at 608.930.7172 and we’ll help you get started.
