Diane Schaaf SWTC Nursing Scholarship
On April 17, 2007 the Upland Hills Health family experienced a tragic loss. On that day, two of our Hospice nurses were involved in an automobile crash. In that accident, one of our long-term nurses Diane Schaaf lost her life. Diane was a beloved employee and friend. To honor her memory and the lasting impact she had at Upland Hills Health, the Board of Directors approved the establishment of an endowment to fund a Diane Schaaf Nursing Scholarship at Southwest Wisconsin Technical College (SWTC). This dedication to Diane, honors a wonderful person, a loving mother & wife, and a dedicated and caring nurse. Diane graduated from SWTC with her associate degree in nursing and also taught wound care classes at the college to aspiring nursing students. The Endowment provides a $500.00 Nursing Scholarship to a SWTC nursing student each school year. The scholarship funds are taken from the net investment income earned on the principal donation set aside for this account; which will provide future scholarships indefinitely. Additional charitable contributions can be made to this fund, by any individual or group. The additional contributions will be used to grow the Account which will allow for more scholarships in the future. If you are interested in contributing to this memorial, you can do so by writing a check out to the Southwest Technical College Foundation C/O the Diane Schaaf Scholarship. Any donations can be dropped off in the Human Resources Department or sent directly to Southwest Technical College. All donations are tax deductible and donors will be recognized by the college.

Upland Hills Health Nursing Scholarship at SWTC
In 2002, UHH started a scholarship endowment with a donation to the school of $10,500.00. Since that time, UHH and the Auxiliary have continued to support the program. Contributions to increase the endowment continue each year. Each year UHH awards a $500 Scholarship to an Associate Degree Nursing Student at Southwest Tech. The Scholarship was begun in an effort to support SWTC and its nursing students, as the college is such an important component to the healthcare services that are offered in Southwestern Wisconsin. The Scholarships from the UHH Endowment are made from the net investment income earned on the account, therefore future scholarships will be available for a long time. Criteria to apply for this scholarship include the following:
Students must have graduated from one of the following high school districts: Dodgeville, Mineral Point, Highland, Iowa-Grant, Barneveld, Pecatonica, Mount Horeb, or River Valley.
Applicants must be in the second year of the Associate Degree Nursing Program with a GPA of 3.0.
Applicants must have completed 34 credits (comparable to second year status) in the A.D.N. program to be eligible.
Applicants must be full-time students.
Current High School Seniors
Upland Hills Health, Inc. – Health Care Services Scholarship Upland Hills Health established this UHH Health Care Services Scholarship program in 2015 to support area high school seniors in meeting their goals to become healthcare providers and to provide support to our area technical college. Two $500.00 Scholarships will be awarded. This is a one-time award, per student, and is not renewable. Applicants must be enrolled and accepted into a health care services degreed or certificate field that is offered at Southwest Technical College (SWTC) located in Fennimore, Wisconsin.

University of Wisconsin Best Rural Health Essay Award
Given by the Hermes Monato, Jr. Memorial Fund The Hermes Monato, Jr. Prize of $2,500 is awarded annually for the best rural health paper. It is open to all students of the University of Wisconsin (any campus) as well as those who will be recent graduates at the time of submission. Students are encouraged to write on a rural health topic for a regular class and then to submit a copy to the Rural Wisconsin Health Cooperative (RWHC) as an entry by June 1st of every year. The writer of the winning essay will receive a check for $2,500.00 paid from a trust fund established at the University of Wisconsin by RWHC, and family and friends of Hermes.
