Community Health Needs Assessment & Action Plan

The Community Health Needs Assessment (CHNA) and Implementation Plan prepared for Upland Hills Health was directed by the CEO and President of Upland Hills Health with contributions from the Iowa County Health Department, the Aging and Disability Resource Center of Southwest Wisconsin (ADRC) and The Community Connections Free Clinic.  The study and data collected was a collaboration between these and other area health-concerned organizations and individuals.
