Palliative Care – It’s about how you live

Palliative Care provides an extra layer of support for people living with chronic illness with the focus of relieving the symptoms, pain and stress of a chronic illness and improving the overall quality of life.

Patients will be seen regularly in their home or clinic by the Upland Hills Palliative Care Nurse Practitioner who specializes in chronic illnesses, managing symptoms and improving quality of life. Palliative Care does not replace the primary care provider and will work closely with them.

With regular visits, patients report that they are able to stay out of the hospital, feel better and do the things that are important to them while living with a chronic illness.

Palliative Care is not Hospice Care.

Palliative Care is available at any time during an illness and can be received at the same time as treatments intended to cure the illness.

Palliative Care focuses on open discussions about future treatment choices and anticipating, preventing and treating symptoms and stressors of illness – whatever the diagnosis may be.

Palliative Care is covered by most insurances.
